For those that were unable to make it to last nights signup please fill out the forms located here http://www.bartlettlacrosse.org/about-us/bartlett-lacrosse-forms/ you will need to complete the Registration, Code of Conduct and Liability Waiver Forms and provide a copy of the players Birth Certificate and Current Physical and a deposit in the form of Check made Payable to Bartlett Lacrosse in the amount of $100.00. If you need to purchase a Uniform you will need to fill out the Uniform Form as well.
Any player that registered and still needs to provide a copy of Birth Certificate or Current Physical you can also mail those forms to the below mailbox.
